What is the mission of VIA Health Partners?Our mission is focused. It is to relieve suffering and improve the quality and dignity of life through compassionate hospice care for those at end-of-life and to provide palliative care for those with advanced or life-limiting illness. We use donors’ gifts to animate this mission across our service area helping to improve the reach and quality of our care not only for patients, but for their loved ones too. At VIA Health Partners we care for all medically eligible patients regardless of medical complexity or ability to pay.

Is VIA Health Partners a new name for the organization?In Spring 2023 Hospice & Palliative Care Charlotte Region, and by extension its two subsidiary corporations, Hospice Cleveland County and Hospice of Laurens County (SC), all became VIA Health Partners. The organization remains a mission-driven, nonprofit healthcare organization that provides hospice and palliative services to sick and dying patients. Hospice & Palliative Care Charlotte Region remains the legal name of the corporation. VIA Health Partners is the corporation’s DBA.

Are gifts to VIA Health Partners tax deductible?Yes. VIA Health Partners is a North Carolina nonprofit corporation recognized under section 501(c)(3) of the Internal Revenue Code. Gifts to VIA Health Partners are deductible to the full extent of the law. When making gifts to VIA Health Partners and other tax-exempt organizations, it’s always a good idea to check with your financial advisor to understand your specific tax implications.

Can I get a list of individuals who donated to VIA Health Partners in memory of my loved one?Yes. We are happy to share a list of donors who chose to honor your loved one with a gift to VIA Health Partners. Our policy is to share the names and mailing addresses and not the specific gift amount. How do I make a gift to VIA Health Partners?Cash gifts can be made three ways: Gifts Online Visit our online giving page. All major credit cards are accepted. Gifts by Phone Call and ask to be directed to one of our Philanthropy Team members at: 833.839.1113 Philanthropy team members are available Monday – Friday Gifts by Mail Send your check to: VIA Health Partners Attn: Philanthropy P.O. Box 470408 Charlotte, NC 28247 Be sure to include the name and address of any honoree you would like us to notify on your behalf if your mailed gift is intended as an honor or memorial donation.

May I designate my gift?Yes. Gifts may be designated regionally to Cleveland County and Laurens County (SC) area or by purpose. Use the dropdown menu on our website or write on the memo line of the check to designate your donation.

How do I make an honor or memorial donation?Tribute gifts provide an import source of revenue for VIA Health Partners. Visit the section above titled, "How do I make a gift to VIA Health Partners?” for detailed instructions. Honor gifts are intended to honor a living friend or loved one. Memorial gifts intended to honor a deceased friend or loved one. In either case, please be sure to include the name and address of any honoree you would like us to notify on your behalf.

Does VIA Health Partners accept gifts of stock, appreciated securities?Transferring stocks and other appreciated securities to VIA Health Partners can be smart tax-planning for donors while helping to animate our patient-centered mission in important ways. To initiate your transfer, follow these two steps: 1. Contact Tom Atwood at 704-335-4325 | atwoodt@viahp.org to inform us of your intent to donate stock. 2. Does VIA Health Partners accept corporate matching gifts?Matching Gifts Yes. Many corporations will match an employee’s gift so long as that gift is made to a qualified public charity like VIA Health Partners. Matching gifts are initiated by the employee on behalf of the charity. Please check with your Human Resources team for more information. Workplace Giving Some companies will host their own workplace giving campaign, often collecting donations through their internal payroll system. Please check with your Human Resources team to see if this is an option for your specific workplace.

How can I make regular or monthly donations to VIA Health Partners?Enroll in VIA Health Partners’ Promise Circle – a group of special donors who contribute monthly or quarterly to help bring our mission to life across our service area. Promise Circle donors either automate their giving through their online bill pay system or keep a credit card on file at VIA Health Partners for scheduled processing. I would like to direct gifts to VIA Health Partners through my loved one’s obituary. What is the proper language I should use?We suggest the following language for obituaries and death notices: In lieu of flowers, the family asks that gifts be made in (his/her first name) memory to VIA Health Partners. Tell me more about Donor Advised Funds and Qualified Charitable Distributions.Donor Advised Funds (DAFs) Establishing VIA Health Partners as a qualified grant-recipient of your Donor Advised Fund is easy to do. Most DAFs will ask for some or all of the following information: Organization Name: Hospice & Palliative Care Charlotte Region (d/b/a VIA Health Partners) EIN: 56-1219017 IRS Status: 501(c)(3) public charity Mailing Address: P.O. Box 470408, Charlotte, NC 28247 Founding: Incorporated in North Carolina on March 8, 1978 Contact: Tom Atwood, Vice President of Philanthropy Please be sure to consult with your DAF holder for specifics details pertaining to your situation. Qualified Charitable Distributions (QCDs) Supporting VIA Health Partners mission through a Qualified Charitable Distribution (QCD) allows individuals who are 70½ years old or older an easy and tax-advantaged way to support our mission. Age-qualified individuals may contribute up to $100,000 total to VIA Health Partners directly from a taxable IRA instead of taking a required minimum distribution. As a result, donors may avoid being pushed into higher income tax brackets and prevent phaseouts of other tax deductions. Please be sure to consult with your IRA holder for specific details pertaining to your situation.

I currently support your organization through my Donor Advised Fund (DAF). Do I need to update my DAF with your new name, VIA Health Partners?No. If you are supporting Hospice & Palliative Care Charlotte Region or one of our affiliate organizations, Hospice Cleveland County and Hospice of Laurens County, your gifts will continue to be processed and acknowledged under those names.

How can I support VIA Health Partner’s patient-centered mission with a legacy gift?Legacy giving is a powerful way to ensure your values live on within organizations that are important to you. VIA Health Partners is honored to count dozens of supporters as members of its Legacy Society. The easiest way to make a Legacy Gift to VIA Health Partners is to: YOUR WILL Make a provision in your will naming VIA Health Partners as a direct beneficiary of a specific amount or percentage thereof. Sample language / codicil : "I give, devise and bequeath to Hospice & Palliative Care Charlotte Region, a North Carolina nonprofit corporation doing business as VIA Health Partners (EIN: 56-1219017), { ____% of my gross estate OR ____% of my residual estate OR the sum of $ ____ to be used for such purposes as the governing body may designate." YOUR LIFE INSURANCE Assign ownership of a paid-up whole life insurance policy (or similar product) to VIA Health Partners. Name VIA Health Partners as a direct beneficiary of a whole life or term insurance policy. YOUR ACCOUNTS Name VIA Health Partners as a direct beneficiary of an IRA, brokerage, savings, money market, or other accounts. By naming VIA Health Partners as a direct beneficiary, in most cases you ensure that those assets are transferred to the organization in a timely manner, bypassing your estate. When it comes to thoughtful legacy planning, VIA Health Partners always encourages donors to seek out professional advice and counsel to ensure the needs of you and your family are being met.
